Form 80 (Personal Particulars for Assessment Including Character Assessment) is one of the most detailed forms in the Australian immigration system. Required for most visa subclasses, it asks for a comprehensive personal history including every address you've lived at, every country you've visited, your employment history, education history, and more — typically going back 10 years or more.
Getting Form 80 right is critical. Inconsistencies between your Form 80 and other documents in your application can delay processing or even lead to refusal on character grounds.
When is Form 80 required?
Form 80 is required for most visa subclasses, including:
- All skilled migration visas (189, 190, 491)
- Employer-sponsored visas (482, 186)
- Partner visas (820/801, 309/100)
- Student visas (500) — for applicants from some countries
- Any visa where DHA requests it for character assessment
Key sections and how to complete them
Part B — Your personal details
Straightforward — name, date of birth, place of birth, citizenship. Ensure your name matches your passport exactly. If you've ever used a different name (maiden name, previous name, alias), list every variation.
Part C — Address history (last 10 years)
List every residential address for the past 10 years, with no gaps. Start with your current address and work backwards. Include:
- Full address (street, suburb/city, state, postcode, country)
- Dates (month/year from and to)
Common mistake: Leaving gaps between addresses. If you moved on 15 March but your new lease started 1 April, you still need to account for those two weeks. Use your most accurate recollection.
Part D — Employment history (last 10 years)
List every job, including part-time work, self-employment, and periods of unemployment. Include:
- Employer name and address
- Your position/title
- Dates of employment
Periods of unemployment should be listed as "Unemployed" with the relevant dates. Do not leave gaps.
Part E — Education history
List all educational institutions attended, including dates and qualifications obtained.
Part G — Travel history (last 10 years)
List every country you have visited in the last 10 years, including:
- Country
- Dates of entry and departure
- Purpose of visit
This is often the most difficult section because people don't keep records of every trip. Check your passport stamps, frequent flyer records, email confirmations, and social media check-ins to reconstruct your travel history.
Part I — Character declarations
Answer all character questions honestly. Questions cover criminal convictions, military service, involvement in organisations, and other matters. A "yes" answer doesn't automatically mean refusal — but a dishonest "no" that DHA later discovers will almost certainly lead to refusal or visa cancellation on character grounds.

- Consistency is everything — Form 80 details must match your Form 1221 (if required), your visa application, and any other documents. DHA cross-references everything.
- No gaps — every day of the past 10 years should be accounted for in your address and employment history.
- Be honest about character questions — DHA has access to international databases. Undisclosed information is treated far more seriously than disclosed information.
- Use exact dates where possible — if you can't remember the exact date, use your best estimate and be consistent across all forms.
- Keep a copy — you may need to refer back to your Form 80 if DHA asks follow-up questions or if you apply for another visa in the future.
